Admission Application Checklist
- ASC Application form and $50 non-refundable application fee
- If you are currently enrolled in a Master's program, submit a program of study signed by your advisor
- Wallet-size photo of yourself
- 3 Letters of recommendation which should speak of your
management and leadership abilities (Required
Form)
- Letters of recommendation for the ASC Program should address your leadership and management skills, not your ability to complete a
graduate program. One letter should be from your superintendent or someone else in authority in your District office, one from your principal,
and one from university/college instructor whom you have had a class from.

- Official graduate transcripts
- Photocopy of Current teaching license ( Level 2 license required. Must have 2 years of certified experience before being admitted to the program.)
- Evidence of your GRE/MAT test score
- USU (Re)Admissions application Online ** (This is done through the undergraduate website.)
** Note: Do not apply for admissions through the Graduate school. **
